Position Description

RN Hospice Case Manager

Location: Milwaukee, WI

Job Type: Contract | Days | Mon-Fri

Contract Length: 13 Weeks with possible extension

Pay Range: $3000 - $3180 Gross Weekly

Key Responsibilities:

Conduct comprehensive patient assessments to develop individualized care plans.
Coordinate and manage patient care in collaboration with physicians, nurses, social workers, chaplains, and other healthcare professionals.
Monitor patient progress and adjust care plans as needed.
Provide emotional support and education to patients and their families.
Advocate for patient comfort and dignity, ensuring they have access to necessary resources and services.

Requirements

Minimum of 2 years of nursing experience, preferably in hospice or palliative care or any acute or nursing experience
RN License
BLS
